The Upstox demat account closure form is a simple 1-page form. Follow the below steps to download the form.
- Visit www.upstox.com and go to Support >> Download Forms
- Scroll to 'Account Modification Forms' section
- Download the 'Demat Account Closure' form
- Print it.
The Upstox account closure form has 4 parts:
- At the top of the form, enter the date and select 'BO' in the Closure Initiated by row.
- Fill account holder details like Client ID, name or names in case of joint accounts, address, city etc.
- Fill in details like a reason for closing the account, the select status of Balance securities remaining in the account, DP ID of the demat account where you want existing securities to be transferred.
- All account holders need to sign on the form.
The second part is to pick a choice for securities held in the demat account and finally the signature of all account holders.
